    上肢の関節可動域が制限される人のためのエプロン提案 : 着脱動作分析による検証

    加齢や障がいにより関節可動域が狭くなると、後ろで紐を結ぶタイプのエプロンは着用が困難となる。本研究では、上肢の動きを制限された人でも簡単に着脱できるエプロンを提案し、着脱時の負担を軽減できるかどうかを検証するため、着脱時の関節の動き、筋肉への負担を一般的なエプロンと比較した。紐を後ろで結ぶ一般的なタイプの「レギュラーエプロン」と市販のエプロンホルダーを使用したワンタッチで着脱できる金城学院ファッション工房の「オリジナルエプロン」を同じ布地で製作した。健康な女子大学生10名を被験者とし、上肢4か所と僧帽筋の筋電図を記録し、筋電積分値から着脱時の筋肉への負荷量を比較した。被験者の前方・右側面の2 方向から着脱動作を撮影し、各ポイント(手首、肘、肩)の上下・左右・前後方向の動きを解析した。着脱時間は、オリジナルエプロンの方がかなり短かった。オリジナルエプロンの筋電積分値は、レギュラーエプロンの10~20%であった。手首の動きをみると、レギュラーエプロンでは、前方と後方だけでなく、上方にも大きく動いているが、オリジナルエプロンでは上下移動はほとんどなく、わずかに後方へ動いているだけであった。各部位の最大移動距離を比較すると、いずれの部位でもレギュラーエプロンの方が有意に大きかった。以上より、オリジナルエプロンは、レギュラーエプロンに比べて小さな動きで、短時間で着脱することができ、筋肉への負担も小さいことが確かめられた

    Prediction of mobilized hematopoietic stem cell yield in patients with multiple myeloma: Usefulness of whole-body MRI-derived indices.

    IntroductionHigh-dose chemotherapy followed by autologous stem cell transplant is the mainstay of treatment for multiple myeloma (MM). The purpose of this study was to evaluate the ability of MRI-derived indices to predict mobilized hematopoietic stem cell yield.Materials and methodsIn this exploratory pilot work, we retrospectively analyzed 38 mobilization procedures for MM. Successful mobilization procedure was defined as a total yield of >4.0×106 CD34+ cells/kg. Univariate and multivariate analyses were performed to identify factors with a significant effect on successful mobilization from among clinical characteristics including number of prior lines of therapy, period from diagnosis to harvest, type of monoclonal protein (M protein); and radiological characteristics including total diffusion volume (tDV), median apparent diffusion coefficient (ADC) of tDV, and mean fat fraction of bone marrow calculated by MRI.ResultsUnivariate analyses showed that relatively poor mobilization was significantly associated with M protein of Bence-Jones type and with median ADC of tDV (P = 0.02 and P = 0.004, respectively). Multivariate analyses using these two indices showed that median ADC of tDV was a significant predictive factor for adequate mobilization (P = 0.01), with an area under the curve of 0.784 (cutoff value, 1.18×10-3 mm2/s; sensitivity, 72.7%; specificity, 87.5%).ConclusionThe present data indicate that median ADC of tDV is a predictive factor for relatively poor mobilization of hematopoietic stem cells in MM patients undergoing autologous stem cell transplant

    Mutation Analysis of CHST6 Gene in Chinese Patients With Macular Corneal Dystrophy

    Japanese Ministry of Education, Tokyo, Japan [07457417, 16591768]Purpose: To examine the carbohydrate sulfotransferase 6 (CHST6) gene in Chinese patients with macular corneal dystrophy (MCD). Methods: Nineteen unrelated Chinese families with MCD, including 24 patients and 3 unaffected relatives, were examined. Genomic DNA was extracted from peripheral blood leukocytes. The coding region of the CHST6 gene was amplified by the polymerase chain reaction, and the DNA fragments were directly sequenced. Fifty unrelated normal Chinese volunteers served as the controls. Results: Eighteen different mutations in the CHST6 gene (including 15 novel mutations) were identified, of which 12 were missense mutations, 5 were nonsense mutations, and 1 was a frameshift mutation. Six families had homozygous mutation, and 13 families had compound heterozygous mutation. None of these mutations were detected in the normal controls. Conclusions: CHST6 mutations may be responsible for the pathogenesis of MCD in Chinese patients. The Q298X mutation detected in 5 of 19 families (6 of 38 alleles, 15.8%) may be the founder mutation in Chinese patients. However, our findings also indicate a high level of allelic heterogeneity of the CHST6 gene in Chinese patients and in other ethnic groups
